This Claim Form should be filled out online or submitted by mail if you had documented out-of-pocket expenses, fraudulent charges, lost time spent dealing with the Data Incident, documented, unreimbursed extraordinary monetary losses as a result of the Data Incident, or you wish to receive a pro-rata cash payment. Checks will be mailed, or electronic payments will be made, to eligible Settlement Class Members if the Settlement is approved by the Court.

Claim Forms must be submitted online or postmarked (if mailed) no later than September 23, 2024. Claims submitted after the deadline will be deemed untimely and may not be accepted.

Where can I find my login information?
If you received a Notice indicating that you are part of the Settlement Class, you will have a 10-character alphanumeric "Unique ID" and a 4-digit "PIN". Both values will be found on the top of your Notice.
